This is a possible future for Sasuke after he battles Itachi. I don't think he'll lose both eyes, just one... well... this is one theory. During the battle, Itachi is able to get one eye and Naruto saves him before he loses the other.
My other theories is turning either Itachi or Sasuke into a Zatoichi type of character where either of them is 100% blind and still crazy powerful.

That's a pretty sick looking version of Sasuke. Mine looks all boring compared to yours XD
